Transition

As children move through the centre we will ensure that the transition is planned and managed effectively by...

Arranging visits for the child two weeks prior to the move.

The key person will introduce the child's parents to the staff in the new room and in particular the child's new key worker.

Staff from the new room will discuss with parents:

  • How the room operates
  • Expectations of the children
  • Room routines
  • Planning procedure etc
  • Storage of personal equipment eg baskets, bags, pockets

The key Person will forward the child's portfolio on the day the child moves room to the new key person, the portfolio is always kept complete and up to date.

A summative report will also be completed and given to the new key person before the child moves areas.

A new child information form will be completed prior to the child starting in the new room.

Any relevant and important information about the child's day to day care must be passed on to the new area eg. special dietary requirements or any health related issues.
